HOW TO GET A REPLACEMENT VACCINATION CARD:
Residents who have lost their COVID-19 vaccination cards can receive a replacement if they were vaccinated in Contra Costa County. To receive a new card, residents can call the county’s COVID vaccine call center at 1-833-829-2626. Call center staff will need to verify the person’s information in the California Immunization Registry. Once verified, a card will be mailed to the person within 48 hours. The county decided to offer this service because it can take up to six weeks to receive a replacement card through the state. State guidance allows people to use photos of their CDC cards for verification purposes when attending events and performances and Contra Costa Health and the California Department of Public Health encourage people to take photos of their vaccination cards in case they get lost.
